Online Scheduling Request

We are currently accepting new patients, including same day appointments for urgent concerns. If you would like to schedule an appointment, you can do so by contacting us at (480) 981-2888 or by using our online appointment scheduling feature.

Prior to Starting

  • Insurance cards (if applicable). Your appointment request will be declined without insurance information unless you are self-pay.

    • Access to your email (for security purposes, you will receive an email with a 4-digit security code to enter during scheduling).

  • Please click the blue icon “REQUEST AN APPOINTMENT” in the bottom right hand corner of the website

Appointment Types

Please select an appointment type that best describes your main complaint. There will be another area where you will be able to write additional details. ​​

Date of Birth

Select the small “triangle” next to the month and year to select the date.

Insurance

If you cannot find your insurance in the pre-populate list, click the blue “ENTER AS TEXT” option.

Appointment Note

If you have a SECONDARY INSURANCE, please include this information here or any additional information you would like our office to know.

*NOTE: We cannot accept patients with a secondary of insurance of Medicaid EXCEPT FOR AZ COMPLETE HEALTH.

Appointment Verification

You will receive an initial email regarding your appointment REQUEST. YOUR APPOINTMENT IS NOT CONFIRMED UNTIL YOU RECEIVE AN ADDITIONAL CONFIRMATION EMAIL OR PHONE CALL. If you have not received a second confirmation email within 3 business days, please call our office to confirm your appointment.
